The Municipality of Athens “takes care” of the elderly with a new innovative program (video)

The Municipality of Athens continues to invest in improving the daily lives of elderly citizens, utilizing modern practices and new technologies in its Multipurpose Municipal Clinics. Through the new innovative program “I Care”, elderly people with chronic health problems such as hypertension, diabetes, cardiovascular and pulmonary diseases, gain access to holistic care close to their homes.

The Mayor of Athens, Haris Doukas , states: “We ensure that free health services reach every citizen, in every neighborhood of Athens. Through our new program, we listen to the needs of the elderly with chronic diseases and emphasize prevention, safety and quality of life. The specialized staff of the upgraded Multipurpose Municipal Clinics of the Municipality of Athens undertakes their continuous monitoring, offering personalized services. The Deputy Mayor for Social Solidarity & Equality, Maria Stratigaki , states: “At the Multipurpose Municipal Clinics, we continue to add new services that go beyond the classic primary health care. In this direction, we created the Memory Clinic and the Smoking Cessation Clinic and designed the specialized 3rd age program ”I Care” aimed at chronically ill people. With an emphasis on vulnerable groups of the elderly, we are creating a support network that combines medical monitoring with psychosocial support ».

The innovation of the “FRONTIZO” program

The “I CARE” program redefines elderly care, putting people at the center and utilizing modern technologies in a simple and accessible way.

  • Human-centered approach: The patient is treated first as a human being, receiving personalized health services that are friendly and easily accessible.
  • Smart use of technology: The program utilizes modern digital tools, specifically designed to be easy to use and understand, avoiding the complexity of electronic bureaucracy.
  • Collaboration with reputable organizations: Implements best practices, leveraging the expertise of leading national and European organizations.

The preparation of the “FRONTIZO” program began in October, with the aim of perfect organization and effective implementation. Since the beginning of 2025, the first elderly patients have been included in the program, which was designed according to the most modern standards of care for the elderly. Each series of participants includes 40 chronically ill people and the duration of the follow-up is one and a half years. The first group started in January, while the second in February, with the aim of gradually including a total of 300 beneficiaries.

The key elements of the program are:

  • Holistic medical and psychosocial approach.
  • A complete electronic file on the patient’s mobile phone, with the corresponding care plan integrated.
  • Patient education about their disease(s) and corresponding behavioral change.
  • Reduction in hospital visits and hospitalizations.
  • Appropriate management of polypharmacy, wherever and whenever possible.
  • Continuous telematic monitoring, training and provision of appropriate support by specialized personnel, regardless of place and time, with the “smart” use of new technologies (virtual visits).
